What this test measures

CBC + ESR combines a standard Complete Blood Count (haemoglobin, RBC/WBC/platelet counts, indices, differential) with the Erythrocyte Sedimentation Rate. ESR measures how fast red blood cells settle to the bottom of a vertical tube in one hour. The rate rises in any condition that increases fibrinogen, immunoglobulins, or acute-phase proteins — making it a sensitive but non-specific marker of inflammation, infection, autoimmune disease, and malignancy.

The two tests give complementary information: the CBC tells you the cell counts and morphology; ESR tells you whether the body is in an inflammatory state. Indian primary care uses this combined panel routinely for fever workup, joint pain (rheumatoid arthritis, lupus, polymyalgia), suspected TB, or unexplained weight loss.

Why it matters

ESR remains widely used in Indian outpatient practice because it is cheap, easy, and helps distinguish "something is going on" from "all is well" when symptoms are vague. A normal ESR in a patient with vague fatigue significantly lowers the probability of serious chronic inflammation. A very high ESR (>100 mm/hr) narrows the differential to a short list — multiple myeloma, temporal arteritis, polymyalgia rheumatica, deep-seated infection (TB, endocarditis), or metastatic cancer.

ESR is not a screening test for healthy adults — its low specificity means many false positives. It is best used to investigate symptoms and to monitor known disease (rheumatoid arthritis, polymyalgia, giant cell arteritis). Modern hsCRP is faster (rises within 6 hours vs 24–48 hours for ESR) but ESR remains useful where chronic inflammation is suspected.

How to prepare

No fasting required. ESR is affected by pregnancy, menstruation, and severe anaemia — disclose these so results can be interpreted in context.

Frequently asked questions

Why is ESR added to the CBC?

CBC tells you about blood cell counts; ESR tells you whether there is generalised inflammation in the body. Together, they help your doctor decide whether vague symptoms (fatigue, low-grade fever, joint pain) deserve further investigation.

Do I need to fast?

No.

My ESR is 60 — should I be worried?

An ESR of 60 is moderately elevated and points to active inflammation, infection, or chronic disease — but the cause matters more than the number. Your doctor will look at your symptoms, CBC, and other markers (CRP, urine, chest X-ray as needed) to narrow it down.

Why is the ESR higher in women?

Healthy women have slightly higher ESR than men due to hormonal differences. ESR also rises further during pregnancy (up to 45 mm/hr is normal in pregnancy) and during menstruation.

Is ESR or CRP better for detecting inflammation?

CRP rises faster (within 6 hours of inflammation) and falls faster, so it is better for acute monitoring. ESR rises slowly (24–48 hours) but stays elevated longer, so it is useful for chronic conditions. They give complementary information.

Can I have a normal ESR but still have a problem?

Yes. ESR is a screen, not a definitive test. Many serious problems (early cancer, viral infection, autoimmune disease in low activity phase) can have normal ESR.

How quickly will I get the report?

CBC results are available in 4–6 hours; the ESR adds ~1 hour. Most NABL labs report combined CBC + ESR within 6 hours.

My ESR is 5 — is that too low?

A low ESR is generally unimportant. It is sometimes seen with polycythaemia (high RBC count) or sickle cell disease. It does not indicate anything to worry about by itself.
