What this test measures
This chemiluminescence immunoassay (CLIA) detects total antibodies (IgM + IgG) against SARS-CoV-2 (the virus causing COVID-19) in serum or plasma. Most assays target the spike (S) protein or its receptor-binding domain — the same target as most COVID-19 vaccines.
The test does not distinguish between antibodies from infection and antibodies from vaccination. It also does not measure functional neutralising activity — that requires a separate neutralisation assay.
Why it matters
India saw multiple COVID-19 waves between 2020 and 2023, with the majority of the population now having either vaccine-induced or infection-induced antibody. Most Indian adults have received at least two doses of Covishield or Covaxin, and many have had natural infection (often subclinical) layered on top.
The total antibody test is used to confirm past exposure or vaccine seroconversion, for epidemiological seroprevalence studies (used by ICMR for national serosurveys), to support a clinical diagnosis of past COVID-19 in a patient with long-COVID symptoms, and for occasional pre-procedure or pre-employment requirements. Routine antibody testing in healthy adults is not recommended for clinical decisions.
How to prepare
No fasting required. The test can be done at any time of day. Mention vaccination history and any prior COVID-19 diagnosis.

Frequently asked questions
Does a positive total antibody mean I am immune to COVID-19?
It indicates past exposure or vaccination, which usually confers some protection. However, antibody levels do not perfectly correlate with protection, especially against newer variants. Vaccination boosters remain useful.
Can this test distinguish vaccine-induced from infection-induced antibody?
Most spike-based assays cannot. Nucleocapsid-protein assays (less commonly used) can identify infection-induced antibody specifically.
My test is negative even though I had COVID-19. Why?
Antibodies can wane over months, especially if infection was mild. Some assays may miss low antibody levels. A negative antibody test does not erase past infection.
Should I test antibody before a booster dose?
Not routinely. WHO and ICMR do not recommend antibody-based decisions for booster timing. Follow your vaccination schedule.
Is total antibody useful for long-COVID diagnosis?
It can support a diagnosis of past infection if RT-PCR was never done, but long-COVID is a clinical diagnosis based on symptoms.
How soon after vaccination does antibody appear?
Most people develop measurable antibody 2–4 weeks after a vaccine dose. Testing earlier can miss the response.
Does a high antibody mean I will not get COVID-19 again?
No. Re-infection can occur even with high antibody, especially with variant emergence. Antibody titre is one factor among many.
