What this test measures

Anti-HCV IgM is the immunoglobulin-M class antibody that some patients produce early in hepatitis C infection. Unlike most other viruses, IgM is not a reliable marker of acute HCV — many patients with acute HCV never develop measurable IgM, and IgM can also appear during chronic disease flares.

For this reason, international guidelines (CDC, AASLD-IDSA, WHO) do not recommend anti-HCV IgM for routine diagnosis. HCV RNA quantitative is the preferred test for early and acute HCV detection because it becomes positive within 1–2 weeks of exposure — well before any antibody class.

Why it matters

Anti-HCV IgM is occasionally ordered in India when acute HCV is clinically suspected (recent jaundice with high-risk exposure) and when HCV RNA is unavailable or unaffordable. A positive result is supportive but not diagnostic; a negative result does not rule out acute HCV.

In current Indian practice, with HCV now curable using DAA therapy under NACO at low cost, the more useful test pathway is anti-HCV total (or IgG) for screening, followed by HCV RNA quantitative for confirmation. IgM has largely been superseded except in specific research or seroconversion studies.

How to prepare

No fasting required. The test can be done at any time of day. Mention any recent high-risk exposure, transfusion, or known HCV contact.

Frequently asked questions

Should I order anti-HCV IgM if I suspect recent infection?

No — HCV RNA quantitative is preferred. It detects infection earlier and more reliably. Anti-HCV IgM has low sensitivity in acute HCV.

What does a positive IgM mean?

It supports recent or active HCV exposure but is not diagnostic. Confirm with HCV RNA quantitative to know whether infection is active.

What does a negative IgM mean if I had a high-risk exposure last week?

It does not rule out HCV — many acute cases never develop IgM. HCV RNA, which becomes positive 1–2 weeks after exposure, is the right test.

How long does IgM stay positive in HCV?

Unlike HAV or HBV, HCV IgM kinetics are unreliable — it may persist intermittently for months in some chronic carriers and may never appear in others.

Why is anti-HCV IgM still offered by some labs?

Historical practice and clinician familiarity. International guidelines no longer recommend it for routine diagnosis, but it is occasionally used in seroconversion studies.

Can the HCV vaccine cause a positive IgM?

There is no licensed HCV vaccine, so this is not a concern.

Is anti-HCV IgM useful for blood donor screening?

No. Indian blood-banking rules require anti-HCV total (or NAT) plus HCV RNA for donor screening.
