What this test measures
A Lipid Profile-Mini measures Total Cholesterol, HDL (often direct), and Triglycerides — and calculates LDL using the Friedewald formula (LDL = Total Cholesterol − HDL − Triglycerides/5). It is the budget-friendly variant of the full Lipid Profile.
The Friedewald formula is accurate when triglycerides are below 200 mg/dL, becomes less accurate between 200 and 400, and unreliable above 400 — in those cases, a full Lipid Profile with direct LDL is preferred.
Why it matters
Lipid Profile-Mini is a sensible cost-effective option for routine cardiovascular screening in adults with no known cardiovascular disease, normal triglycerides on prior testing, and standard risk-factor management. For people with diabetes, established cardiovascular disease, very high triglycerides, or those on lipid-lowering therapy, a full Lipid Profile with direct LDL is preferred — the small extra cost buys meaningful accuracy.
How to prepare
Fast for 9–12 hours (water allowed). Avoid alcohol for 24 hours before. Continue medications unless your doctor instructs otherwise.
Markers & reference ranges
Reference ranges below are typical adult values. Your lab's reported range may differ slightly based on the assay platform and patient demographics — always read your report against the range printed on it.
| Marker | Normal range | If low | If high |
|---|---|---|---|
| Total Cholesterol (mg/dL)[1] | < 200 desirable · 200 – 239 borderline · ≥ 240 high | Generally not concerning unless < 120. | Pair with LDL and triglycerides; full Lipid Profile if borderline / high. |
| HDL Cholesterol (mg/dL) | Men ≥ 40 · Women ≥ 50 | Independent cardiovascular risk factor — common in Indians. | Higher is generally better; ≥ 60 protective. |
| Triglycerides (mg/dL) | < 150 normal · 150 – 199 borderline · 200 – 499 high · ≥ 500 very high | Usually not concerning. | ≥ 500 — pancreatitis risk; lifestyle and medication needed. |
| LDL Cholesterol (Calculated) (mg/dL)[1] | < 100 optimal · 100 – 159 borderline · 160 – 189 high · ≥ 190 very high | Generally good; targets may be much lower if high CV risk. | Friedewald formula becomes unreliable when triglycerides exceed 200 mg/dL — switch to a full Lipid Profile with direct LDL. ≥ 190 suggests familial hypercholesterolaemia. |
Lipid Profile vs Lipid Profile-Mini
| Feature | Lipid Profile | Lipid Profile-Mini |
|---|---|---|
| LDL measurement | Direct (accurate even when TG high) | Calculated (unreliable when TG > 400) |
| HDL measurement | Direct | Direct (in most Indian labs) or calculated |
| Use case | Diabetes, established CVD, high TG, on therapy | Routine screening with normal prior TG |
| Cost | Higher | Lower |
| Accuracy | Higher | Acceptable for screening |
Frequently asked questions
Should I take a full Lipid Profile or the Mini version?
Full Lipid Profile is preferred if you have diabetes, established cardiovascular disease, triglycerides previously above 400 mg/dL, are on lipid-lowering therapy, or have an unusual lipid pattern. Lipid Profile-Mini is fine for routine screening in low-risk adults with no prior abnormalities.
Do I need to fast for a Lipid Profile-Mini?
Yes — fast 9–12 hours. Water is allowed. The triglyceride and calculated LDL values both require a fasting sample to be accurate.
Why is the LDL "calculated" instead of measured?
In the Mini version, LDL is computed using the Friedewald formula: LDL = Total Cholesterol − HDL − Triglycerides/5. This is accurate when triglycerides are below 200 mg/dL, less accurate between 200 and 400, and unreliable above 400 — in those cases the lab will recommend a Direct LDL or a full Lipid Profile.
My triglycerides are 350 — should I upgrade to a full Lipid Profile?
Yes. Calculated LDL becomes less accurate above 200 mg/dL triglycerides and unreliable above 400. A direct LDL (part of a full Lipid Profile) gives a trustworthy value for treatment decisions.
How often should I do a Lipid Profile-Mini?
Every 5 years for low-risk adults from age 20; every 1–2 years if you have any risk factor (BP, diabetes, family history, smoking); annually once you start a statin.
