What this test measures
Thallium is a heavy metal with no physiological role. Historic uses in rodenticides and insecticides are banned in India, but acute thallium poisoning still occurs in industrial accidents, contaminated illicit drug supplies, and very rare criminal cases. Sub-acute / chronic exposure causes the classic triad of GI upset, painful peripheral neuropathy, and alopecia (which typically appears 2–4 weeks after exposure).
Why it matters
Clinical context is everything — order thallium when there is unexplained painful neuropathy plus alopecia, or in cluster GI / neuro illness suggesting common poisoning, or with occupational exposure in semiconductor / optical lens / cement manufacturing. Untreated thallium poisoning is fatal at doses as low as 10–15 mg/kg. Treatment is supportive plus Prussian blue (potassium hexacyanoferrate) which binds thallium in the GI tract and prevents enterohepatic recirculation.
How to prepare
Trace-metal-free royal-blue-top tube for blood; for 24-h urine, collect in a metal-free container, refrigerate, document total volume. Avoid seafood for 3 days (not because of thallium but to keep the trace-metal panel clean if other metals are co-ordered). Disclose any occupational exposure and any recent unexplained illness.

Why does thallium cause hair loss?
Thallium replaces potassium in cells and disrupts keratin formation in hair follicles. Hair loss typically appears 2–4 weeks after exposure and is one of the most characteristic clinical signs.
Is thallium still used in rodenticides?
No — banned in India and most countries since the 1970s for rodenticide use due to severe human toxicity. Current sources are occupational (semiconductor, optical lens, cement), contaminated illicit drugs, and very rare criminal poisoning.
How is thallium poisoning treated?
Activated charcoal if very recent ingestion. Prussian blue (potassium ferric hexacyanoferrate) orally — binds thallium in the gut and interrupts enterohepatic recirculation. Haemodialysis and haemoperfusion can help in severe early cases. Supportive care for neuropathy and GI symptoms.
Should I screen for thallium with vague symptoms?
No — thallium testing is targeted to specific clinical pictures (painful peripheral neuropathy + alopecia, occupational exposure, suspected poisoning cluster). Routine screening on the worried-well is not useful.
Can dental fillings or jewellery contain thallium?
No — thallium is not used in any consumer product. The only common dental / cosmetic source is the rare contaminated batch.
