What this test measures
Total IgE = sum of all IgE antibodies in serum. IgE is the antibody class that mediates immediate (type 1) hypersensitivity — binds to mast cell / basophil receptors and triggers histamine release. Total IgE rises in atopy (asthma, allergic rhinitis, atopic dermatitis), parasitic infections (helminths), some malignancies (Hodgkin's, IgE myeloma), and hyper-IgE syndrome (rare immunodeficiency).
Why it matters
In Indian practice, total IgE is used to: (1) Support clinical atopy when allergy skin tests are unavailable. (2) Screen for parasitic infestation (helminths) when eosinophilia is present. (3) Investigate severe asthma — markedly raised total IgE supports anti-IgE biologic (omalizumab) therapy. (4) Suspected allergic bronchopulmonary aspergillosis (ABPA — usually > 1000 IU/mL). (5) Hyper-IgE syndrome workup in children with recurrent skin / lung infections.
How to prepare
No fasting required. Disclose any current parasitic infestation, asthma medications (oral steroids may transiently lower IgE), recent omalizumab (raises measurable IgE complexed with drug), and ethnicity (Indian / South Asian reference ranges are higher than Caucasian).

Frequently asked questions
Does normal IgE mean I have no allergies?
No — many allergic individuals have normal total IgE. Specific-IgE tests (or skin prick) identify the actual allergen. Total IgE is supportive, not definitive.
My child has recurrent infections and very high IgE — what could it be?
Hyper-IgE syndrome (Job syndrome) is a rare immunodeficiency with very high IgE (> 2000), recurrent skin / pulmonary infections, eczema, and characteristic facial features. Worth genetic / immunological evaluation.
Could a high IgE just be worms?
Yes — helminth infections (ascaris, hookworm, strongyloides, filaria) are common in India and produce eosinophilia plus high IgE. Treat with appropriate antiparasitic and re-check.
Is IgE useful for asthma severity?
It guides biologic therapy. Omalizumab (anti-IgE) is indicated for severe allergic asthma with raised total IgE (typically 30–1500 IU/mL within the dosing window).
What is ABPA?
Allergic bronchopulmonary aspergillosis — an immune reaction in asthmatics to inhaled Aspergillus. Total IgE > 1000 + specific IgE / IgG to Aspergillus + radiology + clinical features confirm it.
Do indians have higher IgE than Westerners?
Yes — Indian / South Asian baseline IgE is often 2-3x Caucasian reference ranges, likely due to higher background parasitic and atopic load. Use Indian reference ranges where available.
