What this test measures

Transferrin is the plasma glycoprotein that transports iron from absorption sites (gut, macrophages) to the bone marrow and other tissues. Each transferrin molecule can carry two iron atoms. Serum transferrin is produced almost exclusively by the liver and is closely regulated — when body iron stores are low, the liver upregulates transferrin synthesis (so levels rise); when iron is plentiful or there is inflammation, transferrin production falls.

The direct transferrin assay measures the actual transferrin protein concentration, whereas TIBC measures the total iron-binding capacity (a calculated surrogate for transferrin). The two correlate closely (TIBC ≈ transferrin × 1.4). Most Indian labs report TIBC rather than direct transferrin because it integrates better with the iron-saturation calculation.

Why it matters

Serum transferrin is used in three settings: (1) Confirming iron deficiency anaemia (transferrin is high — the body is upregulating capture); (2) Differentiating anaemia of chronic disease (transferrin is low — inflammation suppresses synthesis); (3) Workup of suspected haemochromatosis (transferrin is low; transferrin saturation is high).

Direct transferrin measurement is also useful as a marker of nutritional status — it has a short half-life (~8 days, vs ~20 days for albumin), so it falls earlier than albumin in protein-calorie malnutrition. In Indian critical-care and nephrology practice, transferrin is sometimes used to monitor nutritional rehabilitation. In pregnancy and oral contraceptive use, transferrin rises due to oestrogen effect.

How to prepare

Fast for 8–10 hours. Avoid iron supplements for 24 hours. Disclose pregnancy, oral contraceptive use, recent surgery/illness, and any liver disease.
