What this test measures

Serum zinc reflects the small (~0.1%) circulating pool of total body zinc — but it is the most accessible biomarker and is widely used to flag deficiency or excess. Zinc is a cofactor for over 300 enzymes (alkaline phosphatase, carbonic anhydrase, RNA polymerase, alcohol dehydrogenase), is essential for cell-mediated immunity, taste and smell, wound healing, sperm production, and prostate function. Best drawn fasting in the morning — serum zinc dips up to 20% post-prandially and during acute inflammation (zinc redistributes into the liver).

Why it matters

Zinc deficiency is widespread in India — ICMR-NIN and INDIAB studies suggest 30–60% prevalence in rural children, pregnant women, and the elderly, driven by predominantly cereal-based diets (rice, wheat) high in phytates that block zinc absorption. Clinical sequelae of deficiency include impaired growth, recurrent infections, delayed wound healing, dermatitis around mouth and limbs, hair loss, taste/smell disturbance, hypogonadism, and acrodermatitis enteropathica in inherited cases. Supplemental zinc (typically 10–20 mg/day for children, 20–40 mg for adults) is a standard adjunct in diarrhoeal disease (WHO recommendation), in growth faltering, and in stocked Indian public health programmes.

How to prepare

Fast 8–12 hours and collect a morning sample for the most reliable reading. Use a trace-metal-free royal-blue-top tube. Avoid haemolysis (red cells have ~10x serum zinc — even mild haemolysis dramatically raises the result). Stop zinc supplements for at least 24 hours unless you specifically want to measure on-supplement levels. Note any current illness — acute infections, surgery, MI all lower serum zinc as an acute-phase response.

Frequently asked questions

Why do I need to fast for zinc?

Serum zinc drops after a meal (zinc shifts into tissues post-prandially) by up to 20%. A morning fasting sample gives the most consistent reading and best comparison over time.

I have white spots on my nails — am I zinc deficient?

White nail spots (leukonychia) are usually due to minor trauma, not zinc deficiency. True zinc deficiency causes brittle nails, hair shedding, peri-oral and acral skin lesions, recurrent infections, and taste disturbance — not isolated nail spots.

How long does it take for zinc levels to recover?

On adequate supplementation, mild deficiency usually corrects in 4–8 weeks; severe deficiency may need 3 months or longer, especially if the underlying cause (malabsorption, chronic loss) is not addressed.

Can high-dose zinc cause copper deficiency?

Yes. Daily zinc > 40 mg sustained for several months can compete with copper at the intestinal metallothionein binding site and produce copper deficiency, which manifests as anaemia (often confused with B12 deficiency) and myelopathy. If long-term zinc is needed, monitor copper.

Does zinc help with COVID or colds?

Modest evidence — zinc lozenges (75 mg+/day, started in first 24h of symptoms) shorten common cold duration by ~1 day in some studies. Evidence for COVID-19 outcomes is mixed and not currently recommended outside research settings.

Is dietary zinc enough in vegetarian diets?

Vegetarian and vegan diets provide less bioavailable zinc — animal sources (red meat, shellfish) are easier to absorb. Indian vegetarian diets dominated by polished rice, wheat, and lentils with high phytate content can fall short — soaking, sprouting, and fermenting (idli, dosa, dhokla) reduce phytates and improve zinc absorption.
