What this test measures

Beta hCG measures the beta subunit of human chorionic gonadotropin, the hormone produced by trophoblast cells of the developing placenta. It appears in maternal blood as early as 8–10 days after ovulation, well before a missed period, and rises rapidly (roughly doubling every 48–72 hours) through the first trimester. Levels peak around 8–11 weeks at 25,000–290,000 mIU/mL and then decline.

Unlike a urine pregnancy test, which is qualitative (yes/no), a serum beta hCG gives an exact number. Serial measurements (48 hours apart) are used to confirm a viable intrauterine pregnancy, detect ectopic pregnancy, evaluate miscarriage risk, and monitor gestational trophoblastic disease (molar pregnancy, choriocarcinoma).

Why it matters

For most women, beta hCG is the first hard evidence of a pregnancy and the cleanest way to date a very early pregnancy when an ultrasound is still inconclusive. For doctors managing first-trimester bleeding or pain, serial beta hCG values steer the entire workup — a slow-rising hCG suggests a failing intrauterine pregnancy or an ectopic; a falling hCG confirms miscarriage; a plateauing or abnormally high hCG raises suspicion for a molar pregnancy.

In the Indian context, where ectopic pregnancy still accounts for 4–10% of maternal deaths and where access to early ultrasound is patchy, the beta hCG remains the single most useful triage tool in early pregnancy. It is also a screening marker in the Dual / Triple / Quadruple aneuploidy panels, and a tumour marker in germ-cell tumours.

How to prepare

No fasting required. The test can be done at any time of day. Inform the phlebotomist if you have received any hCG-containing fertility injections (e.g. Ovitrelle, Profasi) in the last 10–14 days — these can produce a falsely positive or elevated result. Recent miscarriage or delivery: hCG takes 4–6 weeks to return to non-pregnant levels.

How early can a beta hCG detect pregnancy?

A serum beta hCG can detect pregnancy as early as 8–10 days after ovulation — typically a few days before a missed period. It is more sensitive than a home urine pregnancy test.

Should beta hCG double every 48 hours?

In a healthy early intrauterine pregnancy, beta hCG typically rises by at least 50–66% every 48 hours up to about 6 weeks. A slower rise raises concern for an ectopic or failing pregnancy and warrants ultrasound. After 7–8 weeks, the doubling rate naturally slows.

Can the test give a false positive?

Rare but possible. Recent hCG injections for fertility treatment can cause false positives. Heterophile antibodies can give falsely high readings. Pituitary hCG (after menopause) can cause mildly elevated values without pregnancy.

My beta hCG is dropping — what does that mean?

A falling beta hCG in early pregnancy usually means miscarriage or a resolving ectopic. Your doctor will combine it with ultrasound findings and clinical assessment to confirm and plan management.

What is the difference between beta hCG and free beta hCG?

Total beta hCG is what most pregnancy tests report. Free beta hCG measures only the unbound beta subunit and is used specifically in first-trimester aneuploidy screening (Dual Marker) — it is a more sensitive marker for Down syndrome at 11–13 weeks.

I had a miscarriage — when will my beta hCG come back to zero?

After a first-trimester loss, beta hCG usually drops below 5 mIU/mL within 4–6 weeks. Your doctor may track serial values to confirm complete resolution.

Can men or non-pregnant women have a beta hCG test?
