What this test measures

This is the same biochemistry as the standard Dual Marker — PAPP-A and free β-hCG measured at 11+0 to 13+6 weeks — but the risk algorithm is adapted for twin pregnancies. In twins, both placentas contribute hormones to the maternal circulation, so absolute serum levels are typically 1.5–2× higher than singleton values. Without twin-specific MoM correction, every twin pregnancy would falsely appear "high-risk."

For twins, the risk is calculated separately depending on chorionicity (determined by ultrasound). Monochorionic twins share one placenta and are treated as a single risk per pregnancy. Dichorionic twins (separate placentas) get a risk calculated per foetus, although the biochemistry contribution still has limits — twin screens are less accurate than singleton screens (detection ~75–80% for Down vs 85–90% in singletons).

Why it matters

Twin pregnancies are increasingly common in India due to assisted reproduction (IVF, IUI). They carry higher base rates of structural and chromosomal abnormalities and significantly higher pregnancy risks (preterm birth, growth discordance, pre-eclampsia, twin-to-twin transfusion syndrome in monochorionic twins). Reliable first-trimester screening is therefore even more important — and it requires a lab and algorithm that explicitly handle twin pregnancies.

Choosing the wrong "singleton" Dual Marker for a twin pregnancy will mis-estimate the risk. The "Single & Twin" version is the correct selection if there is any possibility of multiples — even before the NT scan confirms it. NIPT for twins is also feasible from 10 weeks, with caveats around lower fetal fraction.

How to prepare

No fasting required. Schedule the blood draw close to your NT scan (ideally same day or within 1–2 days). The NT report must state chorionicity (monochorionic vs dichorionic) — this is essential for the risk calculation. Disclose IVF/IUI conception, ethnicity, smoking, and prior pregnancies on the request form.

Frequently asked questions

Why is there a separate test for twin pregnancies?

Twin pregnancies produce roughly 1.5–2× more PAPP-A and free β-hCG than singletons. The risk calculation has to use twin-specific medians and incorporate chorionicity, otherwise every twin pregnancy would falsely appear high-risk.

Can I use the standard Dual Marker for a twin pregnancy?

No — the standard singleton algorithm will mis-classify many twin pregnancies as high-risk. Always select the twin-capable version when there is any chance of multiples.

How accurate is the test in twins?

Detection rates are slightly lower than for singletons — about 75–80% for Down syndrome in dichorionic twins, and lower for monochorionic. NIPT is increasingly preferred in twins as the screening sensitivity is closer to singleton-level.

When should I do it?

Between 11+0 and 13+6 weeks, ideally on the same day as the NT scan that confirms chorionicity.

What is chorionicity and why does it matter?

Chorionicity tells you how many placentas the twins share. Monochorionic twins share one placenta (and identical genes), so their risks are calculated per pregnancy. Dichorionic twins have separate placentas and risks per foetus. This affects both the maths and the antenatal monitoring plan.

If the result is high risk, can amniocentesis be done in twins?

Yes, but it is technically harder and the procedural risk is slightly higher than singletons. It must be done by a foetal medicine specialist who can sample each amniotic sac separately. CVS is also possible.

What about higher-order multiples (triplets etc.)?

Biochemical screening is unreliable for triplets and above. Most centres use NT scan + maternal age, or move directly to diagnostic testing if indicated.

Is NIPT available for twins?

Yes, with the caveat that the foetal fraction tends to be lower per foetus in twins, so the no-call rate is slightly higher. Most current platforms validate for dichorionic twins.
