What this test measures

COVID-19 RT-PCR (Reverse Transcription Polymerase Chain Reaction) detects SARS-CoV-2 viral RNA in a sample, usually a combined nasopharyngeal + oropharyngeal swab. It targets multiple viral genes (E, N, ORF1ab, RdRp depending on the assay) to confirm infection.

The result is reported as detected/not detected, often with a cycle threshold (Ct) value. Lower Ct values mean more viral RNA was present at sampling, but Ct values cannot be directly compared across labs or platforms.

Why it matters

RT-PCR is the international gold standard for diagnosing active SARS-CoV-2 infection. ICMR India, WHO, CDC, and NIV Pune all endorse RT-PCR as the reference test. It is required for: - Confirming COVID-19 in symptomatic patients - Pre-procedure or pre-surgical clearance in high-risk patients - International travel (where still required by destination) - Workplace clearance after isolation - Post-exposure assessment of close contacts

During active COVID-19 waves in 2020–2022, RT-PCR was the basis of India's contact-tracing and isolation strategy. In 2026, with most of the population having hybrid immunity, RT-PCR remains the diagnostic standard but is ordered more selectively — primarily for symptomatic patients, hospitalised patients, and specific clinical scenarios.

How to prepare

No fasting required. Avoid eating, drinking, smoking, brushing teeth, or using mouthwash 30 minutes before the swab. Sample is collected via nasopharyngeal swab (a thin swab inserted into the back of the nose); some labs add an oropharyngeal (throat) swab.

Frequently asked questions

How long do RT-PCR results take?

Most NABL-accredited Indian labs deliver results within 6–24 hours. Sample-to-answer can be faster in larger hospital labs.

What does the Ct value mean?

Cycle threshold (Ct) is the number of PCR cycles needed to detect the virus. Lower Ct = more viral RNA. Ct values vary between labs and platforms — they should not be used to make clinical decisions directly.

Can RT-PCR stay positive after recovery?

Yes — non-viable viral RNA can persist for weeks after clinical recovery. A positive RT-PCR weeks after symptoms does not necessarily mean active infection or infectivity.

Is the nasopharyngeal swab painful?

Brief discomfort and a tickling sensation are normal. It is not painful but can cause mild tearing or sneezing.

Do I need RT-PCR for travel?

Requirements have largely eased. Check current advisories for your destination country before booking.

My rapid antigen was negative but I have symptoms. Should I do RT-PCR?

Yes. Rapid antigen tests can miss infection, especially with low viral loads or specific variants. RT-PCR is more sensitive.

How long should I isolate after a positive RT-PCR?

Follow current ICMR guidance — usually 7 days from symptom onset with at least 3 fever-free days. Severe and immunocompromised cases may need longer.
