What this test measures
This plate-based ELISA detects total antibody (IgM + IgG combined) against SARS-CoV-2 in serum or plasma. It does not separate the two antibody classes — a positive result simply indicates that the immune system has encountered the virus or vaccine.
ICMR developed and deployed COVID-Kavach ELISA across India in 2020–21 for national seroprevalence mapping. Most current Indian total antibody ELISAs target the spike protein, with some research assays targeting nucleocapsid.
Why it matters
Total antibody ELISA is used to confirm past SARS-CoV-2 exposure or vaccine response, in epidemiological serosurveys (ICMR national surveys), and occasionally in clinical practice. It is a more sensitive past-exposure marker than IgM alone or IgG alone because it captures any antibody class.
For healthy adults, routine antibody testing is not recommended by WHO, CDC, or ICMR for clinical decisions. The test is most useful in serosurveys, in immunocompromised patient assessment, and to support past-COVID diagnosis where RT-PCR was never done.
How to prepare
No fasting required. The test can be done at any time of day. Mention vaccination history and any prior COVID-19 diagnosis.

Frequently asked questions
What is the advantage of total antibody over IgG?
Total antibody captures both IgM and IgG, so it is positive for a wider time window after infection. For past-exposure detection long after illness, IgG and total give similar results.
Does this test distinguish vaccine from infection antibody?
Most spike-based ELISAs cannot. Nucleocapsid-based assays (like ICMR COVID-Kavach) can identify infection-induced antibody specifically.
Can total antibody diagnose current COVID-19?
No — antibody develops 1–4 weeks after infection. RT-PCR or rapid antigen are used for current infection.
Why was total antibody important in ICMR serosurveys?
Total antibody is highly sensitive for past exposure, making it ideal for population-level seroprevalence mapping. The 2020–21 ICMR serosurveys used COVID-Kavach total antibody ELISA across India.
How long does total antibody stay positive?
Often more than a year after natural infection or vaccination. Decline over time is normal; immune memory often persists beyond measurable antibody.
Should I get this test before a booster?
No. WHO and ICMR do not recommend antibody-guided booster decisions in healthy adults.
Is this test useful in pregnancy?
Routine antibody testing is not part of antenatal care. Used selectively in research or special situations.
