What this test measures
The NS1 antigen rapid test detects the dengue virus non-structural protein 1 (NS1) — a soluble glycoprotein secreted by infected cells from the very first day of fever. Result appears in 15–20 minutes.
NS1 is detectable in blood from day 1 to about day 7 of fever in primary infection. After day 7, NS1 sensitivity drops as antibody clears the antigen — IgM testing then becomes more useful. NS1 sensitivity is somewhat lower in secondary dengue (lower NS1 peak) where IgM rises faster.
Why it matters
In India, dengue causes massive monsoon-season epidemics. NS1 is the earliest reliable marker — it allows confirmation of dengue from the first day of fever, before antibodies develop. This enables earlier hospital admission decisions, fluid management, platelet monitoring, and outbreak control.
NCDC, ICMR, and NIV Pune all recommend NS1 as the first-line test in days 1–7 of fever, often combined with IgM after day 5. NS1 ELISA is the lab confirmation standard but the rapid NS1 has good sensitivity (~80–90%) in primary dengue and is widely used in OPDs and fever camps.
How to prepare
No fasting required. The test uses serum, plasma or whole blood. Mention day of fever onset — affects test choice.

Frequently asked questions
How early can NS1 detect dengue?
From day 1 of fever, sometimes even before symptoms appear. It is the earliest reliable dengue marker.
My NS1 is negative but I have dengue-like symptoms. What now?
If fever started > 5–7 days ago, NS1 may be falsely negative — test IgM instead. In secondary infection, NS1 can be lower. Repeat with IgM ELISA if clinical suspicion is high.
What are dengue warning signs?
Severe abdominal pain, persistent vomiting, mucosal or skin bleeding, restlessness or lethargy, rapid breathing, sudden drop in platelets or rise in haematocrit. These need urgent hospital evaluation.
How accurate is the rapid NS1 test?
Sensitivity ~80–90% in primary dengue, lower in secondary. Specificity is >95%. ELISA NS1 is more sensitive in atypical or secondary cases.
Will NS1 be positive in vaccinated patients?
The dengue vaccine (Qdenga) does not produce NS1 antigen. A positive NS1 always indicates active wild-type infection.
How long does NS1 stay positive?
Usually days 1–7 of fever. By day 8–10, NS1 is rarely detectable.
Can I take aspirin for dengue fever?
No. Use paracetamol only. Aspirin, ibuprofen, and other NSAIDs worsen bleeding risk and should be avoided.
