What this test measures

Anti-HCV IgG is the immunoglobulin-G antibody produced by the immune system against hepatitis C virus. It typically appears within 4–12 weeks of exposure and persists for years to decades, regardless of whether the infection clears spontaneously, is cured by treatment, or progresses to chronic infection.

Most modern anti-HCV "total" assays detect both IgG and IgM together — separate IgG-specific testing is less commonly used clinically. The clinical interpretation is the same: positive antibody indicates exposure; HCV RNA distinguishes active from cleared infection.

Why it matters

India has 6–11 million people with chronic HCV — most undiagnosed. Anti-HCV IgG is the long-term marker of past exposure and the standard screening tool. With curative DAA therapy now widely available in India at low cost through NACO and generic manufacturers, identifying infected individuals matters more than ever.

IgG persists for life even after successful cure — patients should be counselled that a positive IgG after treatment is expected and does not mean the infection has returned. Re-infection is detected by HCV RNA, not by antibody.

How to prepare

No fasting required. The test can be done at any time of day. Mention any transfusion history, tattoos, dialysis, or known HCV contact in the request form.

Frequently asked questions

What is the difference between anti-HCV IgG and anti-HCV total?

In most Indian labs they are essentially the same — modern "anti-HCV" assays detect both IgG and IgM. IgG is the dominant long-lasting antibody.

My anti-HCV IgG is positive. Does that mean I have hepatitis C now?

Not necessarily. Positive IgG means past or current exposure. HCV RNA is required to confirm active infection. About 20–30% of patients clear HCV spontaneously and would still have positive IgG.

How long does IgG stay positive after cure?

Usually for life. After successful DAA therapy, HCV RNA becomes undetectable but IgG remains positive. Re-infection is monitored by HCV RNA only.

How soon after exposure does IgG appear?

Most patients develop detectable IgG within 4–12 weeks. In immunocompromised people it can take longer. HCV RNA is the earliest marker (1–2 weeks).

Can a positive IgG be a false positive?

Modern assays have >99% specificity, but very low-titre reactives without risk factors can be false positives. HCV RNA quickly resolves any doubt — if RNA is undetectable and risk is low, the IgG is likely a non-specific reaction.

Is anti-HCV IgG used to monitor treatment?

No. Treatment response is monitored by HCV RNA — undetectable RNA at 12 weeks after treatment ends defines sustained virological response (SVR), the cure marker.

Does positive IgG mean I am infectious?

Infectivity depends on HCV RNA, not IgG. If RNA is detectable, you are infectious; if not, you are not.
