What this test measures

HSV IgM is the immunoglobulin-M class antibody against herpes simplex virus. Although in theory IgM should rise during primary infection, in practice HSV IgM has poor diagnostic value: - IgM is often absent in true primary infection - IgM can rise during recurrences, not just primary infection - Cross-reactivity between HSV-1 and HSV-2 is common - False positives occur with other herpesvirus infections (EBV, VZV)

For these reasons, the CDC and IDSA do not recommend HSV IgM for routine diagnosis. HSV PCR from an active lesion is the gold standard for diagnosing herpes.

Why it matters

HSV IgM is occasionally ordered as part of TORCH screening in pregnancy, in neonatal sepsis workup, and in some workup of unexplained ulcers. Its interpretation is unreliable, and CDC/IDSA recommend against using it for routine clinical decisions.

When a clinician needs to confirm acute primary HSV infection, the preferred approach is: (1) HSV PCR from any visible lesion or CSF; (2) HSV type-specific IgG testing — seroconversion from negative to positive over weeks confirms primary infection; (3) IgG avidity testing for timing in pregnancy. IgM alone should not change management.

Should I retest IgM in a few weeks?

Type-specific IgG seroconversion (negative to positive over 4–8 weeks) is the better confirmatory approach. IgM retesting is less helpful.

Will treatment depend on IgM result?

Antiviral therapy is based on clinical features and PCR confirmation, not IgM. A positive IgM alone should not start lifelong treatment.
