What this test measures

HSV-1 IgM is the immunoglobulin-M antibody against herpes simplex virus type 1. In theory it should rise during primary HSV-1 infection, but in practice it is unreliable: - Often absent in true primary HSV-1 infection - Can rise during recurrences - Cross-reactivity with HSV-2 and other herpesviruses - Limited specificity

CDC and IDSA do not recommend HSV IgM for routine diagnosis. HSV PCR from a lesion or type-specific IgG seroconversion is preferred.

Why it matters

HSV-1 IgM is occasionally ordered as part of TORCH screening, in neonatal sepsis workup, or when investigating recurrent ulcers — but interpretation is rarely actionable. Modern HSV diagnostic pathways rely on: (1) HSV PCR from a swab of any visible lesion or from CSF in neurological cases; (2) HSV-1 and HSV-2 type-specific IgG (glycoprotein G-based) for past exposure assessment; (3) IgG seroconversion over 4–8 weeks to confirm primary infection.

A positive IgM in isolation should not be used to start antiviral therapy or counsel patients about new infection.

Why is HSV-1 IgM not reliable?
