What this test measures

Mumps IgM (immunoglobulin M) is the first antibody produced after mumps infection. It typically appears 5 days after symptom onset, peaks around 7–10 days, and then declines over 4–8 weeks. A positive IgM is the standard laboratory confirmation of acute mumps infection.

Taken alongside the clinical picture (fever, headache, parotid gland swelling, painful chewing), a positive IgM confirms the diagnosis and helps differentiate mumps from other causes of parotid swelling (bacterial parotitis, salivary stones, lymphadenitis, other viral parotitis).

Why it matters

Mumps is not part of India's national immunisation schedule, so many Indian adults are non-immune and outbreaks in schools, colleges, hostels and workplaces are reported every year. Confirming mumps by IgM is important for: isolating the case (mumps is contagious from 2 days before to 5 days after parotid swelling), tracing contacts, and watching for complications — orchitis (affecting 15–30% of post-pubertal males with mumps), oophoritis, meningitis, pancreatitis, and rarely deafness.

For adolescents and adults presenting with parotid swelling, a confirmed mumps diagnosis also avoids unnecessary antibiotics, which are not effective against a viral illness.

How to prepare

No fasting required. Best taken 5 days or more after symptom onset for highest sensitivity. If sampled in the first few days, an early negative result may need repeating in a week. Mention any recent MMR vaccine — it can produce transient IgM positivity.

When is the best time to take the IgM test?

5 days or more after symptoms begin. Earlier samples can be falsely negative because antibody levels have not risen yet.

Is mumps contagious?

Yes — mumps spreads through respiratory droplets and saliva, from 2 days before to 5 days after parotid swelling. Patients should isolate at home during this window.

I am an adult male with mumps — should I worry about fertility?

Orchitis (testicular swelling) affects 15–30% of post-pubertal males with mumps. Most recover fully, but in a small minority it can lead to reduced sperm production. See your doctor immediately if you develop testicular pain or swelling.

What about hospital-style treatment?

There is no specific antiviral. Management is supportive — paracetamol for pain and fever, soft diet (chewing is painful), warm or cold compresses to the parotid, and rest. Hospital admission is needed for complications.

Does the MMR vaccine affect the IgM result?

A recent MMR can sometimes produce a transient IgM positive. Mention any vaccine taken in the previous 4–6 weeks to your doctor and the lab.

Should my family be tested too?

Only if they develop symptoms. Non-immune household contacts should be assessed for an MMR booster after exposure.
