What this test measures

Varicella zoster virus (VZV) IgM is the first antibody produced after acute chickenpox infection or VZV reactivation (shingles). It appears 3–5 days after the rash, peaks at 2 weeks, and decays over 4–8 weeks. A positive IgM supports an acute or very recent VZV infection.

IgM testing for VZV is less commonly used than for measles or rubella because the diagnosis of chickenpox is usually clinical — the rash and its evolution are characteristic. IgM testing is reserved for atypical presentations, immunosuppressed patients, and obstetric scenarios where confirming the diagnosis changes management.

Why it matters

In pregnancy, a confirmed acute VZV infection (especially in the first 20 weeks) raises the risk of congenital varicella syndrome and warrants specialist care. Severe maternal varicella pneumonia is a real risk in the third trimester. In any of these scenarios, IgM confirmation matters.

In immunosuppressed patients (transplant, chemotherapy, biologics, HIV) VZV reactivation can be widespread (disseminated zoster) and life-threatening. Early IgM and PCR confirmation supports prompt antiviral treatment (intravenous aciclovir).

For public-health surveillance and outbreak investigation in schools, hospitals or hostels, IgM (alongside PCR) confirms cases and guides isolation and contact tracing.

How to prepare

No fasting required. Best taken 3–7 days after rash onset for highest sensitivity. Mention recent VZV vaccination, immunosuppression, or pregnancy to your doctor and the lab.

Frequently asked questions

When should this test be done?

3–7 days or more after the rash starts. Earlier than that, IgM may not yet be detectable. For severe or immunosuppressed cases, a VZV PCR is more sensitive and faster.

Is testing needed if my rash is classic for chickenpox?

Often not — in healthy children and adults the classic rash is enough for diagnosis. Testing is reserved for atypical rashes, pregnancy, immunosuppression, and outbreak investigation.

I am pregnant with chickenpox — what should I do?

Speak to your obstetrician urgently. Antiviral treatment (aciclovir) may be offered. The risk of congenital varicella syndrome is highest in the first 20 weeks; severe maternal pneumonia is a concern in the third trimester.

Can shingles cause a positive VZV IgM?

Yes — VZV reactivation in shingles can produce a rise in IgM in some patients. PCR of a vesicle swab is the most specific test for confirming shingles when needed.

How contagious is chickenpox?

Very contagious — spread by respiratory droplets and contact with vesicle fluid. People are contagious from 1–2 days before the rash until all lesions have crusted (usually 5–7 days). Isolation is recommended during this period.

Should family members be tested?

Not routinely. Non-immune household contacts should be assessed for vaccination after exposure if more than 72–96 hours have passed; VZIG is given to specific high-risk non-immune contacts (newborns, pregnant women, immunosuppressed).
